The header box is four levels deep only in the "Employees involved" block (where "In polls" sits above a "Resulting in deauthorization" / "Resulting in continued authorization" pair, which in turn sits above "Number" / "Percent of total"); the tall cells elsewhere ("Total", "Total eligible", the polls-side "Number"/"Percent of total", and the valid-votes "Total", "Percent of total eligible", "Number", "Percent of total eligible") are repeated down to level 4 as instructed. The caption carries a raised footnote marker 1 tying it to the single footnote, but the caption itself is omitted per instructions. The footnote reads "Sec 8(a)(3)" with no period after "Sec" as printed.
