Reader’s notes

The header box is four levels deep; the deepest nesting occurs only under "Employees involved", so headings that span fewer levels are repeated downward (e.g. "Resulting in deauthorization" in the "Number of polls" group occupies levels 2-3 with "Number"/"Percent of total" at level 4, while "Cast for deauthorization" under "Valid votes cast" occupies level 2 only, with its "Number"/"Percent of total eligible" subheads occupying levels 3-4).
