The last three columns have a two-level heading (the spelled-out name above, the code letters below); the other columns' headings are tall cells, repeated across levels as instructed. Suspected misprint: Arizona, column "All C cases" is printed with a spurious comma as "3,73" and is transcribed that way. Both the row's own components (324+42+6+1) and the Mountain-division column total (1,768) indicate the intended value is 373. The caption carries a superscript 1 and the stub heading "Division and State" a superscript 2, but the corresponding footnote texts are not printed within this crop (no footnote block appears below the table), so `footnotes` is empty. The page is a section of a longer table: it begins mid-list with the East North Central division and ends with the New England subtotal under a double rule, so other divisions and any grand total appear on adjacent pages. Row labels for the division subtotal lines (East North Central, East South Central, Middle Atlantic, Mountain, New England) are indented in the original; indentation is not preserved in the grid. "Rhode Island" is printed on two lines and joined into one label. All rows cross-foot and all division subtotals cast correctly against their member states, which supports the digit readings.

"All cases" and the stub heading occupy the full height of the header, so each is repeated on all three heading levels; "Union deauthorization cases", "Amendment of certification cases" and "Unit clarification cases" occupy the top two levels, with UD/AC/UC on the third. The page is a "—Continued" portion of Table 6A, so the group rows "Outlying Areas", "Pacific" and "South Atlantic" are indented subtotal lines (they carry values) and the table continues onto the following page. No footnote text is printed on this crop, although footnote marker 1 appears in the caption (excluded per instructions) and marker 2 appears on the stub heading "Division and State"; their texts must be on another page. All cells were legible. As an internal check, every row's component columns sum to its "All C cases", "All R cases" and "All cases" entries, and the "Outlying Areas", "Pacific" and "South Atlantic" subtotals equal the sums of their member states in every column, so no misprints are suspected. "District Of Columbia" is printed with a capital O, transcribed as printed.

Printing oddity in the heading: the "All cases" header box carries a stray numeral "230" printed on a third line, below the words "All" / "cases". It duplicates the Iowa row's value in that same column and is evidently a typesetting/layout artifact, not part of the heading, so I transcribed the heading as "All cases" alone. Flagging it here rather than putting it in the cell. The crop is a continuation page (caption reads "—Continued"), so it begins mid-table with the West North Central division states; earlier divisions are on preceding pages. The footnote block is clipped by the bottom edge of the crop: footnote 1 is legible but its descenders are cut, and the footnote keyed to the "2" marker on "Division and State" is not visible at all (it falls below the crop). All internal arithmetic checks out: each state row's "All cases" equals All C + All R + UD + AC + UC, the C and R component columns sum to their "All" columns, and both division subtotals plus the grand total are consistent with their components. No suspected numeric misprints.
