The caption carries a footnote marker (superscript 1 after "Fiscal Year 1989"), but no footnote text is printed within this crop; the footnote list is therefore empty and the marker may be resolved on the page below the cropped area. Column structure: 7 printed columns. "Formal actions taken by type of case" spans the last five columns (Total formal actions taken, RC, RM, RD, UD); a double rule separates RD from UD, and throughout the table the "Total formal actions taken" column equals RC+RM+RD only, with UD excluded (e.g., 851+29+109=989). Suspected misprint: under "Transferred by Regional Directors for initial decision" (4, 4, 4, 0, 0, 0) the two sub-rows "Elections directed" (3, 3, 0, 0, 0, 0) and "Dismissals on record" (1, 1, 0, 0, 0, 0) show 0 in the RC, RM, RD and UD columns even though the parent row shows 4 under RC and the first two columns of the sub-rows sum correctly to 4. The zeros are transcribed exactly as printed. "Review of Regional Directors' decisions:", "Regional Directors' decisions:" and "Outcome:" are label-only section lines with no printed values; the values printed on the line beside "Requests for review received" belong to that row, not to the section heading above it. All other row and column sums in the table are internally consistent, which supports the digit readings. The scan renders some "i" characters faintly (e.g., "in" in the column head, "Regional Directors", "initial"); these are ordinary scan artifacts and the words are transcribed normally.

Seven columns: stub, "Cases in which formal actions taken" (a tall single heading spanning both header levels), then five columns under the spanner "Formal actions taken by type of case" (Total formal actions taken, RC, RM, RD, UD). A heavy double rule separates the RD and UD columns; UD is nonetheless a single ordinary column. In the "Denied" row a small stray slash-like ink mark sits in the gutter between the RM cell (6) and the RD cell (11). It is a scan artifact, not a digit or a footnote marker, and has not been transcribed; the RM value is 6 and the RD value is 11. The arithmetic of the table is internally consistent everywhere I checked (the "total" rows equal the sums of their components in all six value columns), except for one flow relation that a reader may notice: "Request for review received" 203 less "Withdrawn before request ruled upon" 1 is 202, while "Board action on request ruled upon, total" is 206 (and 194 - 1 = 193 against 186, etc.). These are transcribed exactly as printed; the difference is presumably carryover from the prior fiscal year and is not a reading error. Row labels that wrap in print have been joined into one row each: "No exceptions to Regional Directors' reports", "In directed elections (after transfer by Regional Director)", "Review of Regional Directors' supplemental decisions:", and "Withdrawn after request granted, before Board review". The two section lines ending in a colon carry a label and no values. The page is a continuation ("—Continued" in the caption) and the caption, the footnote marker on the caption, and the page number are omitted as instructed. All printed digits were legible; no cell was doubtful.
