Seven columns: stub, "Cases in which formal actions taken", then four columns of actual counts under the spanner "Formal actions taken by type of case" plus its "Total formal actions taken" column. A heavy double rule separates the UD column from RD, and the "Total formal actions taken" column consistently equals RC+RM+RD only, excluding UD (e.g. first row 944+76+165 = 1,185, with UD 4 outside the total; 224+16+28 = 268 for "Requests for review received", UD 2 outside). This is as printed, not an error introduced here — it confirms the column assignment. The caption carries footnote marker 1, but no footnote text appears within this crop; it presumably sits below the cropped area or on the facing page, so `footnotes` is empty. The trailing punctuation on the two unnumbered section lines "Regional Directors' decisions" and "Outcome" is faint in the scan — each reads as a single dot at this resolution, but in context (matching "Review of Regional Directors' decisions:") they are almost certainly colons, which is how I transcribed them. Scanner noise renders several "i" dots faintly ("hearmgs", "decisions", "withdrawn") in the stub; I transcribed the intended words. The last body row ("Dismissals on record", 16/15/13/0/2/0) is followed by a rule, and the table may continue beyond this page.

Arithmetic observations (values transcribed exactly as printed, nothing corrected): the "Total formal actions taken" column consistently equals RC + RM + RD and excludes the UD column throughout, e.g. the first row 962 + 65 + 194 = 1,221 while adding UD 15 would give 1,236; likewise "Board action on request ruled upon, total" 167 + 17 + 12 = 196 with UD 1 excluded. The vertical sums within the "Cases in which formal actions taken" and "Total formal actions taken" columns all check out (602 + 717 = 1,319; 580 + 641 = 1,221; 663 + 54 = 717; 413 + 250 = 663; 26 + 168 + 2 = 196). The section label printed as "Regional Directors' decisions" is followed by a small raised dot that is almost certainly a poorly inked colon (matching "Review of Regional Directors' supplemental decisions:" above); transcribed as a colon. Scattered ink specks appear above the spanner heading "Formal actions taken by type of case", above the RD cell of "Board action on request ruled upon, total" (12), and in the margins; these are page noise, not footnote markers, and were not transcribed.
