The indented stage lines ("Before notice of hearing", etc.) repeat under Consent election, Stipulated election, By withdrawal and By dismissal; they are transcribed in printed order and their wording is not disambiguated. A faint partial line of text is cut off at the very bottom edge of the crop, below the last rule. It is almost certainly a footnote, but too little of it is visible to read, so footnotes is left empty rather than reconstructed.
