p. 255. Three printed columns: the stub (unlabeled in the heading band) plus "AC" and "UC". Only one heading level is printed; the stub heading cell is blank in the image. The scan renders many "i" characters as dotless (e.g. "Certificatıon", "unıt", "dırector's", "decısıon", "Dısmissed", "hearıng", "Wıthdrawn"); these are scan/typeface artifacts and the labels are transcribed with normal spelling. Row structure is indentation-based and several labels repeat ("Before hearing", "After hearing", "By regional director's decision", "By Board decision") under each of the three disposition blocks (Certification amended or unit clarified; Dismissed; Withdrawn). Indentation is not preserved in the grid. Printed totals are internally consistent: AC 8+1+9 = 18 and UC 34+7+27 = 68, matching the "Total, all" row. All digits are single-character and legible; no doubtful cells.
